Many people have been exceptionally kind and helpful during the years in which I have been exploring the themes of this book. I could not possibly name them all, as they cross several continents. But I would particularly like to thank the editors of all the publications which have allowed me to explore the issues raised in this book as well as all the friends, family, antagonists and colleagues who have been willing to discuss aspects of it.

And I would like to dedicate this book to one guide and friend in particular – Stanley.
